P0401
P0401 (generic OBD-II): exhaust gas recirculation (EGR) flow insufficient

Risk level
Medium
Proceed carefully and stop if the check is unclear.
Urgency
Medium
Quick answer
The EGR monitor measured less recirculated exhaust gas than expected. The car usually drives normally, but emissions rise. Read the codes, save the freeze frame and ask about bulletins for your engine before replacing the valve.
Safety first
Stop and check this first
- Replacing the EGR valve without measuring flow or checking bulletins can leave the same code.

What a technician must be able to justify
Technician visit
These are the steps they must carry out and show you. Tick them as they work: if steps are skipped, do not accept a part swap.
These are not instructions for opening or handling the equipment yourself. They are the checks that should support a technical conclusion.
Valve command test and flow data
What they doThe technician commands the EGR valve with the scan tool, watches the pressure sensor or manifold pressure live and reads the monitor result (mode 6 if available).
They must show youthe sensor response when the valve opens and the monitor value against its limit.
In writingwhether the valve, sensor or passages are cleaned or replaced, and why.
Sensor hose check and verification
What they doThe technician checks the EGR pressure sensor hoses and connector and, after the repair, lets the monitor complete again.
They must show youthe state of the hoses and the EGR monitor as complete.
In writingthe repair done and the date the monitor completed with no code.
Before accepting a part or repair
Nothing has been shown yet. Do not accept a part swap based on the code alone.
Follow these steps
Note: Follow the documented order. An error code identifies the affected system, but it does not prove by itself which part has failed.
Codes that come with P0401
- Who:
- you, with a generic OBD-II reader and the car parked.
- How:
- read all codes and save the freeze frame. Note any EGR pressure sensor, manifold pressure or MAF codes.
- Result that matters:
- whether P0401 is alone or comes with codes for the sensors the monitor uses.
- If there are codes for those sensors:
- the workshop must fix them first, because Ford needs them fault free to measure flow.
- If P0401 is alone:
- the next step is measuring flow and checking the valve and passages.
- Evidence to keep:
- a photo of the codes and the freeze frame.
Do not clear the codes and do not use the reader while driving.

How the car drives
- Who:
- you, driving normally.
- How:
- notice any jerking, rough idle or loss of power, besides the light.
- Result that matters:
- whether the car behaves differently from usual.
- If there are clear symptoms:
- book soon and tell the workshop, as there may be another fault besides the EGR.
- If it drives normally:
- this fits low EGR flow; book without urgency.
- Evidence to keep:
- notes on the symptoms, when they occur and since what date.

Service bulletin for your VIN
- Who:
- you, asking your brand's authorised service.
- How:
- give your VIN and code P0401 and ask whether there is a bulletin or engine computer update for your engine.
- Result that matters:
- whether a bulletin matches your model, year and engine.
- If it matches:
- the workshop should apply it before replacing valves or coolers.
- If there is no bulletin:
- that route is ruled out; a full EGR flow diagnosis is needed.
- Evidence to keep:
- the bulletin number or the answer that there is none, with the date.

What it means
P0401 is the generic code for insufficient EGR flow. The EGR system returns part of the exhaust gas to the intake to cut emissions. In Ford's OBD summary the monitor runs once per drive cycle and calculates a flow ratio using the EGR pressure sensor or manifold pressure; the sensor circuit and hose tests must pass first. Ford also published a diesel bulletin where P0401 was fixed by reprogramming the engine computer. The code does not say which part has failed.
Warnings and stop conditions
- Mediumeditorial warning
Replacing the EGR valve without measuring flow or checking bulletins can leave the same code.

Symptoms and causes
Probable causes · in order
- Blocked EGR passages or valvePossible
Carbon build-up reduces gas flow even if the valve moves. It shows when the sensor responds but the measured flow is low.
Evidence: Ford 2020-2023 gasoline OBD system operation
- EGR pressure sensor or its hosesPossible
Ford requires the sensor circuit and hoses to pass their tests before measuring flow. A loose hose or a sensor that does not read skews the result.

- Engine computer strategyLess common
On the 2017 to 2019 Ford F-Super Duty 6.7L diesel, Ford attributes P0401 to software strategies and fixes it by reprogramming. Only if the bulletin matches.
